The document consist of 3 А4 sheets . On the first page positioned a draft of the seal of the Ivano-Frankivsk regional administration of the Ukrainian Association of Political Prisoners and Repressed. On the second sheet positioned the previous design and handwritten in blue ink: “Emblem of the symbols and letterhead” and in black ink: “Project by Uhryn Stepan”. The last sheet is a handwritten letter in blue ink. The letter begins with the words: “Dear Mr Gubka!” and ends: “God bless you! Sincerely, 6-07-93”.
